We are now Essex Internal Medicine!
Dr Michael Baldwin, Dr Alan Spagnola and I formed Essex Internal Medicine in 2010. Our goal was to improve quality and service by embracing up to date information technology. In 2011 Essex Internal Medicine was recognized by the Medicare program as one of the first practices in Connecticut to achieve the lofty federal goals of advanced medical information management. At the same time the group was recognized by the National Committee for Quality Assurance (NCQA) as achieving the highest level of care using the Patient Centered Medical Home model for primary care services . Health information found on the site has been carefully reviewed. Links are provided only to high quality sites maintained by reputable organizations and institutions. High priority has been given to sites that are based on the best available evidence for the information given.

Resources for staying in your home as you age. This site is also valuable for spouses, children and other caregivers.

Office forms can be downloaded. The most commonly used forms can be filled in on your computer and then saved and printed.
House calls. The site reflects a commitment to preserving and expanding a disappearing way of providing care, the house call. If you or someone you know finds travel difficult please review the information on the home visit page.

- View your personal health record. You can use this to inform other physicians including emergency room staff.
